GRADUATE PROGRAM GUIDELINES: PH.D. PROGRAM CHECKLIST

It is the responsibility of the student to meet all degree requirements on time. Graduate School and department regulations deal primarily with time limitations for various steps in the sequence for obtaining a degree. Procedures are provided in the Graduate Bulletin. If you have questions, see your Major Advisor or the Graduate Coordinator.

  1. ________ Report plan to remove deficiencies, if any, through courses during first year of enrollment to Graduate Coordinator.
  2. ________ Major Professor appointed before the end of the second semester of residence. (Failure to identify a satisfactory Major Professor may result in termination.)
  3. ________ * Advisory Committee and Meeting (Major Professor and four faculty members) appointed by the end of the first year; If a co-Major Advisor is appointed, then a sixth person will be added to the Advisory Committee. The department requires submission of a short (1-2 paragraph) statement of your prospective research area. This will aid the GSPC in evaluating the appropriateness of the proposed committee.
  4. ________ * Preliminary Program of Study on approved form submitted to the Graduate Coordinator by the end of the first year of residence. The department requires submission of a brief (1-2 paragraphs), more specific description of your prospective research project to accompany the proposed Program of Study. The form is signed by all members of the Advisory Committee and the Graduate Coordinator. Include 3 or more semester hours of CBIO 9000. Note that prospective doctoral candidates are required to establish residency (check-off below) by registering for 30 consecutive hours of course work listed on Program of Study (exclusive of research skills requirement). Not registering during summer semester does not constitute a break in residency.
  5. ________ Establish Residency Requirement through completing required 45 hours in consecutive quarters. Research skills do not count towards residency requirement. Within first 6 years.
  6. ________ * Written Comprehensive Examination completed by the end of the second year, not including summer semester; results are reported to the Graduate Coordinator.
  7. ________ * Dissertation Prospectus submitted before the Final Program of Study is submitted. Must be on file with the Graduate Coordinator before Orals are scheduled. The Dissertation Prospectus Approval Form needs to be signed by all committee members.
  8. ________ * Final Program of Study on approved form submitted to the Graduate School by the end of the second year. The form is signed by all members of the Advisory Committee and the Graduate Coordinator.
  9. ________ * Oral Preliminary Examination notification to Graduate School, via the Graduate Coordinator, at least two weeks before the date of the examination.
  10. ________ * Oral Preliminary Examination completed by the end of the second year, not including summer semester); results are reported to the Graduate Coordinator and Graduate School.
  11. ________ * Application for Admission to Ph.D. Candidacy filed with the Dean of the Graduate School following completion of requirements. Note that this form must be submitted to the Graduate School by November 1 for June graduation and by the first week of classes of Winter Quarter for August graduation, Spring Quarter for December graduation, and Summer Quarter for March graduation. Must be admitted to candidacy 2 semesters (quarter) but summer can count and a total min. 10 hours of dissertation or other appropriate credit.
  12. ________ Candidate must register for at least three semester hours of dissertation (CBIO 9300).
  13. ________ After admission to candidacy, candidate must register for 2 additional semesters and for any other quarter in which student will be using University facilities. Enrollment for the quarter in which degree requirements are completed is mandatory.
  14. ________ After admission to candidacy, candidate must register for 3 hours for each semester in which the student will be using University facilities. Enrollment for the semester in which degree requirements are completed is mandatory.
  15. ________ * Application for Graduation must be filed in the Graduate School (Room 552)two quarters (Friday of the first full week of classes) in advance of the quarter in which you plan to graduate. The Registrar's Office will mail you a bill for the Graduation fee; be sure to use your campus address and to pay this bill in the time specified. Deadline for paying the fee is 3 days before graduation.
  16. ________ * Dissertation approved by the Major Professor (who notifies the Graduate Coordinator) and submitted to the Advisory Committee at least five weeks prior to the final oral examination.
  17. ________ Dissertation submitted to the Advisory Committee five weeks (recommended)prior to the expected date of the final oral examination. Dissertation must be typed as outlined in instruction booklet available from the Graduate School and written in the style and format acceptable to a scientific journal.
  18. ________ The department requires that all Ph.D. candidates present his/her final defense as a departmental seminar. Notify the Graduate Secretary, two weeks in advance, of the date, time, place and title of dissertation. Announcements will be sent out.
  19. ________ Final oral examination administered by the Advisory Committee and the Major Professor.
  20. ________ * Actions of the Advisory Committee reported to Graduate School on approved form.
  21. ________ The original of the dissertation, signed by the Major Professor and taken to the Graduate School (Room 552) for checking of the format. This must be done no later than TWO weeks prior to graduation. (Do not make official copies before this step, to save time and expense in the event there are corrections to be made.)
  22. ________ Binding fee paid for a minimum of 4 official copies (see below) at the Treasurer's Office; see Graduate School Bulletin for other requirements.
  23. ________ Clean xeroxed copied of the dissertation taken to the Graduate School for the Dean's approval. (Required minimum of 4 copies: 2 for the library; 1 department; and 1 Major Professor.) This must be done no later than ONE week prior to graduation. At this time, also submit to the Graduate School: A binding fee receipt from the Treasurer's Office; a microfilm fee receipt from the Treasurer's Office; two extra copies of the title page; one extra copy of the approval page; a Microfilm Contract (obtained from the Graduate School); a Survey of Earned Doctorates Questionnaire (obtained from the Graduate School); and if copyrighting, a certified check or money order, payable to the University Microfilms.
  24. ________ All official copies of the dissertation, approved by the Graduate Dean, carried to the Main Library (the periodicals desk in the annex basement). At this time, also submit the following to the library: A binding fee receipt; two extra copies of the title page; a copy of the abstract; a statement from the Graduate School that the microfilm fee has been paid; a Microfilm Contract (obtained from the Graduate School); and, if copyrighting, a certified check or money order payable to University Microfilms, and a Microfilm/Copyright Contract (obtained from the Graduate School).
  25. ________ Final clearance: All of the above requirements for the Ph.D. degree completed and reported to the Graduation School, including submission of the final copy of the dissertation, at least ONE week prior to graduation.
