All club members must sign a waiver and present it to the club secretary. The waiver is available in several formats, PDF, RTF, SXW, and MS Word.
For more information on the different document formats.
USFA provides a number of documents online. We have directly linked to a few of the more commonly requested ones. For the complete selection see the USFA website forms section. All the documents on the USFA website require Adobe
PDF documents require the free Acrobat Reader. Most computers will already have it installed but if you don't you can easily download it for free from Adobe. It does not usually let you edit the document, but does preserve the exact formatting.
RTF documents open in most word processors including, MS-Word, WordPerfect, and OpenOffice.org. The document is editable but can only handle basic formatting.
SXW documents require the free OpenOffice.org office suite. Highly reccommended, it is a full featured replacement for MS Office and WordPerfect Office. The program is a large download, however it is free, no catch. The files are editable and retain all the formatting.
MS Word documents only open in Micrsoft Word. They usually have a DOC extension which is actually a generic extension and can cause some confusion. The files are editable and retain all the formatting.
