Application Instructions
Part I: Application Information
Full Name: Enter your complete
name (no nicknames, please)
Address: Enter your mailing address (typically
street address, apt. number, etc.)
City: Enter your city
State: Enter the two letter
abbreviation for your state (e.g., GA)
Zip: Enter your
five-digit zip code
Phone: Enter your phone number,
including area code
Gender: Press the radio button
to indicate either male (M) or female (F)
Email: Enter
your email address, if available
Date of Birth: Enter
your birth date by month, day, and year
Intended Major: Enter your intended major; if undecided,
enter "undecided"
Part II: Learning Community Preferences
There are 8 Learning Communities planned for the 2008-2009 academic year with 20 students in each of these year-long Learning Communities. We ask that you indicate your top 2 Learning Communities for membership application, with #1 being your most preferred Learning Community. If you have only one preferred choice you need not indicate a #2. Please make only one choice as #1 and only one choice as #2.
Part III: Printing Your Application

Print two (2) copies of
your completed application. You
should print both copies before exiting
the Acrobat
Reader application, because you
typically
cannot save the filled-out application to
a file.

Each printed copy should consist of two pages.

Incomplete applications may not receive full consideration.

Keep one copy of the printed application for your personal
files.
Part IV: Mailing the Application
Mail one copy of the printed application to the address
indicated on the bottom of the first page of the
